Managing patients with chronic noncommunicable diseases (NCDs), which account for the majority of morbidity and mortality among the population, is a key challenge for modern healthcare systems. Effectively controlling such conditions as cardiovascular morbidity, diabetes, and chronic respiratory diseases requires both one-off interventions, as well as a continuous, comprehensive, and personalized approach. The primary healthcare system bears the brunt of the long-term management of such patients by treating flare-ups, monitoring patients’ health, making adjustments to their therapy, preventing complications, and providing motivation for lifestyle changes. The continued integration of comfortable and minimally invasive drug administration methods into the healthcare system is key to ensuring that patients follow their prescribed therapy over the long-term, which is crucial to achieving treatment goals. This approach, along with the implementation of joint projects between the pharmaceutical industry and non-governmental organizations, helps to create additional infrastructure solutions, improve the quality of life of patients with chronic conditions, and increase their overall life expectancy. Which modern models of outpatient monitoring and risk management have proven to be effective? How can telemedicine technologies and patient education programmes be integrated into routine practice to improve adherence to treatment? What role do interdisciplinary teams, including physicians, nurses, psychologists, and social workers, play in building a continuous care system for patients with NCDs? What role can joint non-governmental organizations and business projects play in improving the quality of life of patients with NCDs?
