One of the top goals in the healthcare system in the coming years is to ensure the continuous care and monitoring of patients, which is crucial given the global trend of an aging population and, as a result, the increasing prevalence of chronic diseases. Healthcare experts have proposed including chronic obstructive pulmonary disease, which is becoming increasingly common in Russia and around the world, in the list of socially significant diseases, which would improve the quality of medical care and drug provision for such patients. Effective models for managing patients with chronic diseases should include innovative methods with remote diagnostics and monitoring using technologies from the Internet of Things and big data analysis: electronic daily health trackers, wearable devices, and mobile applications. What are the most effective modern approaches to organizing medical care for patients with chronic diseases? How can the use of digital technologies help improve the quality and life expectancy of such patients? What successful projects for managing patients with chronic diseases exist in various regions of Russia? How can international experience and cooperation in combatting chronic diseases, including chronic obstructive pulmonary disease, help modernize patient management models in Russia? What transformations need to take place in the healthcare system to help patients with chronic diseases?
